PRIMARY JOB DUTIES AND RESPONSIBILITIES
PATIENT CARE
- Recognizes abnormal, pertinent findings, and initiates referrals or consultations accordingly
- Conduct pre-transplant interviews with prospective transplant patients and families. Interpret eligibility criteria for patients, families, physicians, and outside agencies
- Prepare and analyze pre-transplant clinical data and collaborate with other medical personnel in preparation of patients and families for transplantation
- Coordinate pre-transplant preparations
- Ensure patient and family education in all aspects of transplantation
- Compose written transplant summaries and clinic visits
- Coordinate post-discharge planning and education for transplant patients
- Compile and review post-discharge data for clinical management of transplant patients
- Provide direct care to transplant patients in the Outpatient Transplant Clinic
- Collaborate with medical faculty, other transplant coordinators, nursing personnel, case management, social workers , transplant pharmacists transplant dieticians, and other health disciplines to facilitate meeting patient and family needs. Coordinate and lead the transplant selection meetings, document the decisions made by the team
- Provide on-call service on evenings, weekends, and holidays when the transplant Services and Clinics are closed
- Responsible for completing charge capture education and in-service training for their Institute within CCAD. When charges are not captured automatically, the nurse will complete within Epic in real time, Perioperative Services within 24 business hours
QUALIFICATION & EXPERIENCE REQUIREMENTS
- minimum of a Bachelor’s degree in nursing is required; A Master’s degree in Nursing is preferred
- Postgraduate/certification in a relevant nursing specialty is preferred
- 5 years of clinical nursing experience
- Progressive nursing experience with demonstrated success in the relevant nursing specialty (for example: caring for transplant patients in ICU or ACU)
- Clinical nursing experience in the ACU and ICU is preferred
- Two (2) years of relevant nursing specialty experience (example: transplant) is preferred
- valid and current national license is required
- BLS is required
- Strong oral and written communication skills
- Proficiency with Microsoft Office suite
- Strong interpersonal skills
- Ability to effectively manage multiple responsibilities simultaneously
- Fluency in written and spoken English
- Experience with electronic medical records is strongly preferred
- The ability to speak and write in Arabic is preferred
